I would fit NJM4580E-TE1. The usual cause of failure is incorrect power supply, otherwise they are protected against overload.

Thanks for the suggestion of the Nisshinbo part, the internal transistor layout looks identical to the CoolAudio one.

Please check where in diagram that 4580 is. This opamp has one good characteristic to be able to drive very low R loads (down to 100ohm) and Behringer is using that characteristic sometimes.
If its on such place best replace it with new 4580, that's very cheap opamp.
