Amp powers up and produces + - 49 volts on the outputs So im getting rail voltages..
I have + - 15 volts on all opamps..
What is happening is when the amp is powered up the power led fades out and i get no audio out of this amp..
When the power led fades i still get rail and regulated voltges but no audio output..
Anyone have any suggestions on where to start looking ?

Lift D7 and without load or input signal/audio power up the amp and check for excessive heat at the output devices.

Pin 4 should be -15vdc and Pin 5 should be +15vdc and Pin 10 should be -20 vdc (muting). You indicated in Post #1 that you had the +/-15vdc on opamps. Please recheck and verify. You may try using speaker ground as your reference.
Also, measure between Speaker and Power grounds with an ohmmeter

Please explain what is difference between Post #10 and Post #12 results. Did you use power ground for the black probe on #10 and speaker ground for the #12?
Also, please measure with an ohmmeter between speaker and power ground

I was a bit rushed when I posted earlier. I just need to verify what reference(black probe on speaker or power ground?) you used when measuring DC voltages for both Post #10 and #12, and did you measure between speaker and power ground?
This will help explain why there is a discrepancies between both sets of results.

I used the speaker ground in both posts..
In post 10 i think what happened is i didnt have the meter probe pushed down enought in the connector for the readings ..
In post 12 i pushed the meter probe down a lil further so it would make contacct.. i just measured again and all the readings from post 12 is what i still have now..
From power ground to speaker ground i get 0.04 ohms

Okay, that makes things a little more clear! Sometimes the zero ohm resistor/ jumper tying speaker & power ground together will open and cause unexpected voltages readings.
The -13.3 volts on pins 9 & 12 is most likely the issue and the source is IC1-NJM2068.Check DC voltages around the IC and post.
Also, with no power applied measure the resistance between pins 1 & 2 of U1 and while holding the probes rotate the gain control through its entire range, verify that the resistances vary between zero and ~20Kohms.

It appears the IC is bad.Although check the resistors and capacitors around IC1 and if all appears to be in value replace the IC.

Replaced IC 1 an checked he resistors cap all around that IC..
