Am I looking at the orientation correctly? Are pin 5 and 8 directly connected?

Isn't pin 8 the +12v input to the IC?

The pin configuration looks like it could be an LM211 but that seems like an odd IC to have for a non-switching stage in the amplifier. Hopefully, someone else will have a more definitive answer.

For IC2, I'd expect an op-amp like a TL072 or possibly an LM6172.

I'd use the original IC2 unless you find that it's defective. Then, if it works, you can experiment with others in that location.
