Hello Vex,
pl check if you have connected input ground (SG in your layout) to psu ground.
use a separate wire from SG to make connection to psu ground.

@HRDSTL I have quite a few ksc1845 and ksa992 if you want to PM me. I personally would not trust a NTE without comparing the datasheet very closely, the specs tend to be generic and not specific to the task like ksc1845 would be

Sure, Attached last versions.
Version 3.1 with Schurter DD22 inlet in the right side and Version 4.1 with the Schurter DD22 inlet in the left side. just a Reminder this board DOESN'T HAVE SOFT START.
If You would like Soft Start You should choose this one low voltage On-Off switch drives AC mains relay \ includes soft start .. H9KPXG or this one conform to EU regulations i have an update that i can PM if you like

. Different specification. 4570 not for audo applications. The best OPAMP for sound: OPA1641, OPA1611 (single) OPA1656, LME49710 (dual) IMHO.

Generally, 0.1uF or 100nF bypass caps can be ceramic X7R or MKS/MKP is fine. No need for expensive (and polarized) tantalum here.
